CONTRACT HEALTH SERVICES
This is a Federal Program that will assist tribal members in obtaining
health care services through area providers for medical and dental
services. The member must meet all the eligibility
criteria established by Indian Health Service, the Bemidji Area
office for IHS and MBPI.
To apply for Contract Health Services (CHS), you will need to
contact the Health Director for MBPI, complete all required registration
forms which will be submitted to the Bemidji Area Indian Health
Services (IHS) for pre-approval. The Tribal Health
Director will be happy to assist you in this process. The Tribal
Health Director or the Contract Health Services Officer will then
notify you if you are approved or denied for services.
Enrolled MBPI members may also use CHS to assist in co-pays for
prescriptions. We are currently using 3 local pharmacies. You
must notify the Tribal Health Director if you would like to utilize
this. You will still be required to meet the same guidelines as
previously stated above.
IHS SPECIAL GRANTS FOR DIABETES PROGRAMS
This is a federal program offered to tribal members if you are
a diagnosed diabetic or are at risk for developing diabetes. The
goals are:

To increase the Indian communitys knowledge
regarding diabetes self-care and the preventions of complications.

To increase the communitys knowledge regarding
the appropriate level that should be provided to all individuals
with the diagnosis of diabetes.

To determine the prevalence of diabetics within
the MBPI community and the adequacy of care received.

Establish a community blood sugar screening
& diabetic foot screening with referral and follow-up services.

Develop ongoing community diabetes support and
education group.

Inform and assist tribal members of IHS programs
that are available.
We are able to provide a variety of services through the program
such as: glucose monitoring machines, test strips, nutritional
education, and diabetes education. We will notify and provide
glucose screening to members and advertise this through the tribal
newsletter and flyers of special events. Contact the Tribal Health
