CONTRACT HEALTH SERVICES (CHS)
This is a Federal Program that will assist tribal members living within the Contract Heath Service Delivery Area (CHSDA), which are Allegan, Barry, Kalamazoo, Kent and Ottawa counties, in obtaining health care services through area providers for medical and dental services. The member must meet all the elgibility criteria established by the MBPI CHS Department.
To apply for Contract Health Services (CHS), you will need to contact Kelly and complete all required registration forms. Once eligibility has been determined you will be notified if you are approved or denied. Kelly will be happy to assist you with any necessary paper work needed.
Eligible CHS members may also use CHS to assist in co-pays for prescriptions. We are currently using 3 local pharmacies. You must notify Kelly if you would like to utilize this. You will still be required to meet the same guidelines as previously stated above.
