Diabetes Outreach Worker
Carol has worked for the tribe since January, 2002. Her job duties are to assist tribal members in the Diabetes Program, via enrolling into program, patient education of use of glucometers and strip distribution. She also provides transportation to attend monthly support group meetings.

This year, Carol has attended a 3 week training course for the Community Health Representative Program and is now certified as a CHR. She does outreach to tribal members and assists with transporting to appointments as requested by the membership.

Carol currently works part-time, her hours are Monday - Thursday 8:00 am to 2:00 pm.

IHS SPECIAL GRANTS FOR DIABETES PROGRAMS
This is a federal program offered to tribal members if you are a diagnosed diabetic or are at risk for developing diabetes. The goals are:

  • To increase the Indian community’s knowledge regarding diabetes self-care and the preventions of complications.
  • To increase the community’s knowledge regarding the appropriate level that should be provided to all individuals with the diagnosis of diabetes.
  • To determine the prevalence of diabetics within the MBPI community and the adequacy of care received.
  • Establish a community blood sugar screening & diabetic foot screening with referral and follow-up services.
  • Develop ongoing community diabetes support and education group.
  • Inform and assist tribal members of IHS programs that are available.
